Most men should pay more attention to their health.  Men’s health issues often go neglected because men are more likely to smoke and drink.  Men are more likely to make unhealthy or risky health choices.  But men are LESS likely to seek routine medical visits.

Many men’s health issues only affect men.  For example, low testosterone (Low T), erectile dysfunction (E.D.), and prostate cancer are issues that only men have to deal with.  By and large many men’s health concerns are embarrassing and difficult to talk about.

Erectile Dysfunction

By definition this occurs when a man cannot get or keep an erection firm enough to have sexual intercourse.  Of course this men’s health issue is of particular concern for our male patients.  In addition, erectile dysfunction can be a sign of a physical or psychological condition. Furthermore, it can cause stress, relationship strain, and low self-confidence.  Physical conditions that can cause erectile dysfunction include:

  • Heart disease.
  • Clogged blood vessels (atherosclerosis)
  • High cholesterol.
  • High blood pressure.
  • Diabetes.
  • Obesity.
  • Metabolic syndrome — a condition involving increased blood pressure, high insulin levels, body fat around the waist and high cholesterol.
  • Parkinson’s disease.

Erectile Dysfunction Treatment

Oral medications available for Erectile Dysfunction include:

  • Sildenafil (Viagra)
  • Tadalafil (Cialis)
  • Vardenafil (Levitra, Staxyn)
  • Avanafil (Stendra)

This medications enhance the effects of nitric oxide which is a chemical in the body that relaxes muscles in the penis.  This increased blood flow allowing you to have an erection.  Many men think that these tablets will automatically produce an erection but this is incorrect.  Sexual stimulation is needed to cause the release of nitric oxide.  The most common side effects with these medications include: flushing, nasal congestion, headache, visual changes, backache and stomach upset.  Patients with the following disease states / risk factors should not take these erectile dysfunction medications:

  • Take nitrate drugs — commonly prescribed for chest pain (angina) — such as nitroglycerin (Minitran, Nitro-Dur, Nitrostat, others), isosorbide mononitrate (Monoket) and isosorbide dinitrate (Dilatrate-SR, Isordil)
  • Very low blood pressure (hypotension) or uncontrolled high blood pressure (hypertension)
  • Severe liver disease
  • Kidney disease that requires dialysis

The Feared Tri-Mix Injection

Most men do not like the idea of injecting something into their penis but it is very popular among anyone who has ever tried it.  Why is it so popular you ask?  Well, first off it works really well.  We have had many patients who do not even have a prostate see great results while using a penial injection.  Secondly, unlike the oral medications, you do not need sexual stimulation for these medications to work; these will give you an erection regardless.  Lastly, pain is very minimum due to the fine, short needle.

With this method, you use a fine needle to inject alprostadil, papaverine, and phentolamine (aka Tri-Mix) into the base or side of your penis.  The goal is to have an erection that lasts one or two hours.  All in all, the minimal pain is well worth the results.  Your physician and compounding pharmacist will work with you to figure out the right dose and concentration.

    1. You have to get a prescription from your physician. Have him write you a prescription for sildenafil 20mg tablets. So if you need 100mg, you take #5 of the 20mg tablets. Or, you can have your physician write you a prescription for sildenafil 50mg or 100mg troches. If he writes you a Rx from 50 or 100mg, the prescription must say troches. It cannot say tablets.
